The format for doctor's excuse notes is divided into 7 parts: Medical Facility Name and Address in the top of the format, the second part for Greeting depending on who will be getting the note, the third part for Your Name or name of person the note is used by, The reason for the excuse (illness, injury, etc.) in the fourth part, the fifth part for what Doctor will do or has done, the sixth part for the dates you have been or will be absent or will return, the final part for any limitations when you return if any (light duty etc.)

After following the format, you should pay attention to the eight doctor's excuse notes

1. A Podiatrist (foot doctor) Note - This is great for foot problems of any sort. Unique new logo.

2. A Medical Absence Report - Unique in shape and design, this great note has a Christian theme. It's content is similar to the other note. Its layout is unique. A must see.

3. An Oral Surgery Form (with watermark) - Excellent newer addition to our collection. Includes a great logo and watermark. Great for impacted teeth, root canals, wisdom teeth removal, cavities, etc.

4. A "Return to Work" Form - This form stands out from the rest of the pack. It is focused on "returning to work" and when the patient is capable of doing so. Includes a section for follow-up appointments and a large comments / restrictions field.

5. A Classic Medical Excuse - This hospital excuse includes fields for arrival and discharge dates. Very unique in appearance. Can be customized to any field of medicine. Excellent note, probably one of our best.

6. A Prescription-Style Doctor Excuse - About 25% of all doctors write their "excuse notes" on prescription slips. This note looks remarkably legitimate.

7. An Eye Doctor Note (with watermark) - This note as a great watermark and very modern, corporate looking logo. Great for eye exams, pink eye, eye infections, scratched corneas. Just come back with an eye patch. :)

8. A Dermatologist Note (with watermark) - Great for any skin related issue. Acne, boils, extreme sunburn, melanoma and other types of cancer, etc.
